Many of us grew up placing a hand on the left side of our chest when we think about our heart. It feels natural — but is it actually correct? The Heart Is More Central Than You Think

The heart is not completely on the left side of the chest. In reality, it sits in the centre of the chest, tucked safely behind the breastbone (the flat bone running down the middle of your chest). Doctors call this central space the mediastinum.

So why do we always say the heart is on the left?

It's All About the Tilt

While the heart's position is central, it is slightly tilted. Because of this tilt, the lower tip of the heart — known as the apex — points toward the lower left side of the chest.

This is the part of the heart that moves the most with every beat. That rhythmic tapping you feel on your left side? That is the apex gently knocking against your chest wall with each heartbeat. So the sensation is on the left, even though most of the heart sits in the middle.

Why Does This Matter for Chest Pain?

This anatomy explains something patients often ask about: why chest pain or discomfort is usually felt on the left side or in the centre of the chest. Because the apex is tilted left, and because heart-related nerve signals can radiate in that direction, pain tends to be felt more prominently there.

However, heart-related chest pain can also be felt:

  • In the centre of the chest (behind the breastbone)
  • Spreading to the left arm, jaw, neck, or upper back
  • Occasionally even on the right side

This is why it is important never to ignore chest discomfort just because it does not feel exactly like what you expected.

A Simple Way to Remember It

Think of the heart as a slightly tilted fist sitting behind your breastbone — its knuckles (the apex) nudge toward the left with every beat. Centre in position, left in sensation.
